More and more principals now require a TB test as part of the seafarer medical check-up. For crew with a tight joining schedule, the first question is always: which TB test gives the fastest results? The answer: yes, there is one — but each test works differently and has different requirements. Here is the guide.
Why Do Principals Require a TB Test?
A ship is an enclosed space with limited air circulation — an ideal environment for TB transmission. Indonesia is a high-TB-burden country, so many shipping companies, P&I clubs, and flag-state authorities require TB screening as part of crew medical fitness (MLC). The goal is twofold: protecting the crew on board and avoiding quarantine problems at destination ports.

Where Can You Get a GeneXpert TCM Test in Indonesia?
Unlike blood tests available at private clinical laboratories, GeneXpert TCM machines in Indonesia are mostly distributed through the national TB control program: at referral community health centres (puskesmas), public hospitals (RSUD), and TB referral hospitals. As of January 2023 there were 2,187 TCM machines across 34 provinces and 496 districts/cities — and the number keeps growing.
This means you generally need a doctor’s referral: visit the nearest puskesmas or public hospital and ask for the TCM (molecular rapid test) service. The machines are prioritized for TB suspects (with symptoms or X-ray findings), so for routine medicals without symptoms, the usual combination remains chest X-ray + IGRA/BTA as required by the principal.
One practical note: TCM needs a sputum sample. People without a cough often struggle to produce an adequate sample — it must be deep sputum (deep cough), not saliva. If no sputum can be produced, the doctor will usually direct you to a different test combination.
Important Note for Indonesian Seafarers: The BCG Issue
This is what causes the most panic: almost all Indonesians received the BCG vaccine as children. BCG can cause a false-positive Mantoux — even in perfectly healthy people. According to the CDC, the IGRA blood test is not affected by BCG and is preferred for vaccinated individuals.
So if your Mantoux comes back positive: don’t panic yet. Ask for confirmation with an IGRA or chest X-ray, and discuss it with a doctor. A single test result is not a TB verdict.
Tips Before the Test
- Ask the crewing/principal first: which test do they require? Don’t choose on your own — follow their requirements. If they just say ‘TB test’ without details, IGRA + chest X-ray is the fastest screening combination most widely accepted.
- Bring supporting documents: vaccination history, previous medical results, and a referral letter if you have one.
- If you have symptoms, don’t wait for the scheduled medical. Cough lasting more than 2–3 weeks, unexplained weight loss, night sweats, or coughing blood — get examined immediately.
- A reactive result is not the end of your career. TB is curable with complete treatment. What’s dangerous is hiding symptoms just to pass the medical.
